Health Connections Inc. and Koinonia Family Development Center and Church are hosting a COVID & Flu Vaccine Clinic.

• There is no out-of-pocket cost to you. Options are available for the uninsured. Free roundtrip Uber rides will be available. Contact Health Connections Inc. at 414-999-1099 for more information.

• All eligible individuals will be vaccinated regardless of ID, documentation or insurance status

• Vaccines are available for everyone 6 months and older while supplies last.

Important Information:

• On September 12th, 2023, The US FDA approved the new version of COVID-19 vaccinations for everyone age 6 months and older regardless of prior COVID-19 vaccinations

• Currently, Flu vaccines are available for everyone age 6 months and older

• For children ages 6 months - 17 years old, a parent or legal guardian must be present to provide consent

• Please bring your ID, proof of insurance (if available). Your insurance will be billed, and you will not receive a separate bill
